2.  Your age.

3. Your gender identification and sexual orientation (for purposes of group balance; all are welcome).

4.  The Workshop or Group you're interested in.

5.  Whether the proposed time/s works for you or if you cannot make the proposed time/s, or no time has been proposed, what your schedule would allow  (please be very specific about days of the week, specific dates and times as makes sense for the group you are interested in).

6.  A little about yourself and why this group/workshop interests you.

7.  Your previous and/or current experience (if any) with therapy, group therapy or other relevant activities.

8.  Whether the fee indicated is manageable for you or, if it is not, a specific amount that you could afford and a little about your economic situation.

9. Whether you have any special concerns about the group (age, gender, sexual preference make-up? concerns about content?).

10.  If you do not know Susan already, are you willing to speak by phone or meet for a free consultation to discuss whether the workshop is right for you; when are you available to do so?
